Otosyfilis is een vorm van neurosyfilis. Kenmerkende klachten van otosyfilis zijn gehoorverlies, duizeligheid, tinnitus en evenwichtsstoornissen. Syfilis (of lues) is een soa die wordt veroorzaakt door de spirocheet Treponema pallidum en kan zich op vele manieren presenteren. Otosyfilis kan zowel in de vroege als late fase van syfilis optreden. Syfilis wordt bevestigd via serologische non-treponemale en treponemale testen. Bij een (genitale) ulcer kan PCR-onderzoek ingezet worden om T. pallidum-DNA te detecteren. Bij otologische of andere neurologische klachten moet laagdrempelig een intraveneuze behandeling met benzylpenicilline gestart worden. Aanvullende liquordiagnostiek is mogelijk, maar niet vereist voorafgaand aan de start van de behandeling. Liquordiagnostiek wordt tevens niet meer geadviseerd ter follow-up als therapiemarker; serologische controle volstaat. Otosyfilis is een diagnostische uitdaging, belangrijk om tijdig te herkennen en adequaat te behandelen en hierdoor de kans op re-infectie en restklachten (zoals blijvend gehoorverlies) tot een minimum te beperken.
